The Health Education Division of the Middletown City Health Department provides
programming and health information to all citizens of Middletown. The division is grant
funded and serves as fiscal agent for the following grants:

• Child & Family Health Services Grant (CFHS)
• Ohio Infant Mortality Reduction Initiative Grant
• Immunization Action Plan (IAP)

The Division includes a Health Programs Administrator, one Health Educator, one
Community Care Administrator, and three Community Care Coordinators. With a focus
on prevention, they develop, implement and evaluate health programs designed to
reduce health risks of the citizens in Middletown.
