Health and Vital Statistics

The Middletown City Health District is dedicated to preserving, protecting, and improving the health of residents of the City of Middletown. Our mission is to provide those services, as mandated by the statutes of the State of Ohio and/or required by the OHIO PUBLIC HEALTH COUNCIL'S minimum standards for local health departments, in a courteous, efficient, effective, and cost-effective manner.

Within the Middletown Health Department is Vital Statistics which issues certified birth and death records. For events that occur within the Middletown City Limits. The cost of these records is $20.00 each.

The Environmental Health Division inspects and licenses food services, mobile home parks, tattoo facilities, swimming pools and septic systems. This section also investigates nuisance and dog bite complaints within the city limits.

The Health Education Division offers information to the community regarding a wide variety of health issues and programs.
